Youth Program Application

Please be prepared to provide information about the youth applicant's identity, tribal enrollment or descendancy, age, income, education, career interests, and the types of services being requested.

You will be asked to upload supporting documentation, including a photo ID, tribal enrollment or descendancy documentation, income documentation or proof of public assistance, and documentation needed to verify age. Having these documents available before you begin will help you complete the application more efficiently.
