Commodity Program Application

To apply for the Commodity Program, you will need to download the application below. Once you have downloaded the fillable PDF, use your device to fill out and sign the PDF.  You can submit your application by uploading it and your supporting documents via the form on the Submit Your Application page.

Alternatively, you can print the downloaded PDF and fill out the form with a black or blue pen. You can then scan and submit your files or simply deliver or mail them to United Tribes.

In addition to the application, you will also need to choose the appropriate Food Form to fill out and submit. Choose the Food Form that reflects the number of people in your household. Scroll to the bottom of this page to find and download the appropriate Food Form.
