LIHEAP Application

To apply for LIHEAP, you will need to download the application below. Once you have downloaded the fillable PDF, use your device to fill out and sign the PDF.  You can submit your application by uploading it and your supporting documents via the form on the Submit Your Application page.

Alternatively, you can print the downloaded PDF and fill out the form with a black or blue pen. You can then scan and submit your files to liheapcomm@outlook.com or simply deliver or mail them to United Tribes.

If you have zero income, you will need to fill out the Zero Income Form in addition to the application. Scroll to the bottom to download the Zero Income Form.

Liheap is for winter months. If not all documents are submited it will not be processed.
